Answers to the agreed questions, the material considered, the medical reasoning and relevant limitations. An incomplete file may require further information or an examination.
Who it is for
Law firms, insurers, third-party administrators and private clients with a defined need for medical analysis. The intended recipient and purpose determine the scope.
How to start
Send the purpose, preferred specialty, report language and any procedural deadline. Do not send health details or records in the form. After acceptance of scope and fee, we explain document transfer.
Independent conclusions
We do not guarantee a particular conclusion, compensation, benefit entitlement or acceptance by a court or insurer. A private opinion does not replace an official assessment.
We can request records on your behalf
If records are missing, we can request them directly from hospitals, clinics and physiotherapists in Poland on the basis of your written authorisation and release from professional confidentiality. The form is in Polish because it is addressed to Polish healthcare providers; it can be completed on screen or by hand.
