Define the issue
Identify the question, intended use, assessment period and deadline. We can discuss whether the material supports a records-based assessment or whether more evidence is required.
Agreed deliverables
The scope, questions, specialty, language, fee and timetable are agreed before commissioning. Translation and any examination are separately clarified.
Professional independence
The physician assesses the evidence independently. We do not promise a litigation outcome or a particular medical conclusion. Enquiring does not suspend procedural deadlines.
We can obtain records from providers
If your client’s records are incomplete, we can request them directly from healthcare providers on the basis of an authorisation and confidentiality release signed by the client. You can pass the form to your client to complete on screen or by hand; it is in Polish because it is addressed to Polish providers.
