Why does this matter?
Treatment across several providers or countries can leave records fragmented. Poland’s Patient Rights Ombudsman explains patients’ right to access their medical records.
What should you prepare once the scope is agreed?
A chronology of providers and treatment dates, discharge summaries, test results and previous opinions relevant to the agreed questions. Identify each document’s source. The physician determines which records are needed; not every document is relevant to every case.
How does INEM help?
We first establish the purpose and specialty. After scope and fee approval, we explain how to transfer records. The physician identifies gaps rather than filling them with assumptions. The enquiry form is for organisational information only.
